This form must be completed when a Chappelle Gardens Residents Association (CGRA) member wishes to extend their booking privileges to a non-resident.

By completing this form, the resident acknowledges that they are sponsoring a booking on behalf of a non-resident and accepts responsibility for the booking in accordance with CGRA policies and the Chappelle Gardens Rental Agreement.

This is a member privilege and may be revoked at CGRA's discretion.

RESIDENT DECLARATION

By signing below, I confirm that:

I am a current member of the Chappelle Gardens Residents Association (CGRA) in good standing.
The booking listed below is being made on behalf of the non-resident named on this form for their use and benefit.
I understand that I will be designated as the Lead Contact and primary operational contact for this booking and must remain present for the full duration of the event while guests are onsite.
I understand that I am required to complete and sign the applicable Chappelle Gardens Rental Agreement for this booking.
I understand that this booking remains subject to all terms, conditions, fees, policies, procedures, and responsibilities outlined in the applicable Chappelle Gardens Rental Agreement.
I understand that it is my responsibility to ensure the non-resident host receives and is made aware of all applicable CGRA policies, procedures, rules, expectations, and information relating to the booking.
I understand that the non-resident host is responsible for completing payment obligations and submitting any required Damage Deposit Authorization Form.
I understand that the non-resident host is intended to be the primary financially responsible party for rental fees, damages, overtime charges, staffing charges, administrative costs, cleaning fees, policy violations, fines, and other applicable fees arising from the booking.
I acknowledge that if the non-resident host fails to meet these obligations, CGRA may pursue recovery from me as the Resident Guarantor.
I understand that submission of payment or a Damage Deposit Authorization Form does not transfer Lead Contact responsibilities, booking ownership, or resident booking privileges.
I understand that failure to comply with CGRA policies may result in consequences to my membership, including suspension or revocation of booking privileges.

ACKNOWLEDGEMENT

I have read and understand the terms outlined above and agree to act as a Lead Contact and Resident Guarantor for this booking in accordance with the Chappelle Gardens Rental Agreement.
