Damage Deposit Authorization Form

I hereby authorize the use of my credit card as a damage deposit in connection with a rental booking at Chappelle Gardens Residents Association (CGRA) on the date listed below.

This authorization is provided in connection with a confirmed rental booking and is subject to the terms, conditions, fees, and policies outlined in the Chappelle Gardens Rental Agreement(s).

AUTHORIZATION

I understand and agree that:

• My credit card information will be securely stored for the duration of the rental.
• No charge or pre-authorization will be applied at the time of booking.
• Charges will only be applied where damages, policy violations, overtime use, additional staffing, cleaning, administrative costs, or other applicable charges are incurred.

I authorize CGRA to charge my credit card for any applicable costs resulting from the rental, including but not limited to:

• Additional rental time, overtime staffing, unauthorized occupancy, delayed checkout, or use of the facility beyond the scheduled rental period
• Damages to facility structures, furnishings, equipment, décor, amenities, or property
• Cleaning charges resulting from prohibited items, excessive mess, or additional cleaning requirements
• Storage, handling, disposal, or removal of items left behind following the event
• Emergency response costs resulting from the actions or conduct of the Lead Contact, guests, or event attendees
• Policy violations, fines, penalties, or other charges arising from the booking
• Repair, replacement, administrative, or other costs associated with the booking
• Any additional fees or charges outlined in the applicable Chappelle Gardens Rental Agreement(s)

I understand that many incidental charges may be minor in nature; however, I remain responsible for the full cost of applicable damages, fees, repairs, replacements, or policy-related charges, even where such costs exceed $500.

If the cost to repair or replace any individual item, or group of items, exceeds $1,000, CGRA will obtain three independent quotes to help ensure fair market pricing before completing repairs.

ADDITIONAL ACKNOWLEDGEMENT

I understand and agree that:

• If I am submitting this form on behalf of a renter or booking holder, I understand that I am accepting financial responsibility for applicable charges arising from the booking.

• I understand that submitting this form authorizes CGRA to recover applicable charges from me as the cardholder in accordance with the Chappelle Gardens Rental Agreement(s) and related booking documents.

• I understand that providing payment information or submitting this authorization does not transfer booking ownership, Lead Contact responsibilities, resident privileges, or responsibilities established under applicable CGRA booking documents.

• I understand that charges may arise as a result of actions or conduct by guests, attendees, or event participants and are not limited solely to damages personally caused by me.

CARDHOLDER RESPONSIBILITY

I acknowledge that:

• This authorization is legally binding and connected to the associated booking and applicable Chappelle Gardens Rental Agreement(s).
• Charges may be applied in accordance with the terms, conditions, fees, and policies outlined in the applicable Chappelle Gardens Rental Agreement(s).
• CGRA may alternatively issue an invoice for applicable charges. Unpaid balances may be charged to the authorized cardholder if payment is not received within the timeline specified on the invoice.
• If I am not the Lead Contact for this booking, I confirm that I have been informed of the nature of the event and associated responsibilities and voluntarily agree to assume financial responsibility in accordance with this authorization.
• I understand that submission of payment information does not transfer or alter the responsibilities of the Lead Contact or, where applicable, the Resident Guarantor.
• I understand that charges may be applied regardless of whether I personally attend or participate in the event.

PRIVACY STATEMENT

All credit card information will be kept strictly confidential and used solely for the purpose of recovering costs related to damages, violations, overtime use, administrative costs, fees, or other charges associated with the booking as outlined in the applicable Chappelle Gardens Rental Agreement(s).

Once it has been confirmed that no charges are required following post-event inspection and resolution of any outstanding matters related to the booking, all credit card information will be securely destroyed.
