Crowd Pleasers Dance Camps, Inc.
Participant Waiver, Release of Liability, and Media Consent
This waiver must be completed by a parent or legal guardian before a participant may attend or participate in any Crowd Pleasers Dance Camps, Inc. program or event, including but not limited to camps, clinics, workshops, Intensify programs, competitions, and overnight events.
1. Participation Acknowledgment
I understand that my child is voluntarily participating in activities conducted by Crowd Pleasers Dance Camps, Inc. (“Crowd Pleasers”). Activities may include dance instruction, choreography, leadership training, physical conditioning, recreational activities, team-building sessions, and travel between event locations.
I understand participation is elective and that my child participates at his/her own risk.

2. Assumption of Risk
I acknowledge that participation in dance and athletic activities involves inherent risks, including but not limited to:
- physical injury such as sprains, fractures, or strains
- illness or exposure to communicable diseases (including but not limited to influenza, COVID-19, MRSA, and other viral or bacterial infections)
- equipment or facility hazards
- accidents during recreational or outdoor activities
I knowingly and freely assume all risks, both known and unknown, associated with my child’s participation.

3. Release of Liability
In consideration of my child being permitted to participate in Crowd Pleasers programs, I agree to release and hold harmless Crowd Pleasers Dance Camps, Inc., its owners, directors, officers, employees, staff members, contractors, volunteers, affiliates, sponsors, and venue partners from any and all claims, demands, or causes of action arising from my child’s participation.
This includes claims related to injury, illness, disability, death, or loss of property, even if such injury or damage arises from the negligence of Crowd Pleasers or its affiliates, to the fullest extent permitted by law.

4. Medical Treatment Authorization
I authorize Crowd Pleasers staff or designated event representatives to obtain emergency medical treatment for my child if necessary.
I understand reasonable efforts will be made to contact a parent or guardian prior to treatment when possible. I agree to be financially responsible for any medical expenses, transportation, hospital services, or related care.

5. Medical Information Disclosure
Please list any medical conditions that may affect participation, including but not limited to allergies, asthma, diabetes, epilepsy, heart conditions, or physical limitations.
6. Lodging & Hotel Responsibility
Some Crowd Pleasers programs include overnight lodging at hotels, cabins, dormitories, retreat centers, or similar accommodations.
I acknowledge and agree that:
- Participants must respect all lodging facilities and property.
- Any damage to hotel rooms, cabins, dorms, or lodging facilities caused by my child may be the responsibility of the participant, parent/guardian, and/or the associated school or organization.
- Crowd Pleasers Dance Camps, Inc. is not financially responsible for damage caused by participants.
- Lodging providers may charge the participant’s school district, studio, team, or parent/guardian for damages, excessive cleaning, or property loss.

7. Third-Party Venue Acknowledgment
I understand that many Crowd Pleasers events take place at third-party venues such as schools, retreat centers, hotels, convention centers, and recreational facilities.
These facilities are owned and operated by independent entities. Crowd Pleasers does not control or maintain these facilities and is not responsible for their condition or operation.

8. Venue & Facility Conduct
Participants must respect all event venues and facilities including dressing rooms, rehearsal spaces, classrooms, performance areas, and lodging facilities.
Damage to facilities or equipment may result in financial responsibility assigned to the participant’s school, studio, or parent/guardian.

9. Transportation Disclaimer
Transportation to and from Crowd Pleasers events is the responsibility of the participant, parent/guardian, school, or studio unless otherwise specifically arranged.
Crowd Pleasers Dance Camps, Inc. does not supervise or provide transportation and is not responsible for incidents occurring during travel to or from events or lodging locations.

10. Personal Property
Crowd Pleasers Dance Camps, Inc. is not responsible for the loss, theft, or damage of personal belongings brought to any event, camp, rehearsal space, venue, or lodging facility.

11. Code of Conduct
Participants are expected to behave respectfully toward fellow dancers, directors, coaches, staff members, and venue personnel.
Crowd Pleasers reserves the right to remove any participant from activities if behavior is unsafe, disruptive, or violates program expectations. No refunds will be issued in such cases.

12. Social Media and Photography Release
I grant permission for Crowd Pleasers Dance Camps, Inc. to photograph or record my child during program activities.
These images and videos may be used for promotional materials, educational resources, marketing, websites, and official Crowd Pleasers social media accounts.
I understand that no compensation will be provided for the use of these materials.
13. Parent/Guardian Certification
I certify that I am the parent or legal guardian of the participant listed below. I have read and fully understand this waiver and voluntarily agree to its terms on behalf of myself and my child. I acknowledge and accept the terms of this waiver for the period from May 1, 2026, to September 1, 2026.
