Every child must have a waiver signed by their parent or legal guardian before entering the bouncy house. No signed waiver, no entry.
Safety rules
I will explain the rules to each child and help them follow posted rules and attendants' instructions. Participation is limited to children ages 4 through 12. Maximum of 6 children. No food or drinks. No horseplay, flips, wrestling, pushing, or shoving. Slide feet first only. Follow age, size, weight, and occupancy limits. A responsible adult must remain nearby. Safety rules are strictly enforced. Anyone who does not follow the rules may be denied entry or required to leave the attraction. Activities may pause or close for equipment inspections, maintenance, weather, or other safety concerns. All participants must exit promptly when directed by an attendant.
Permission and acknowledgment of risks
I am the parent or legal guardian of each child named on this form and give permission for each child to use the bouncy house at this event. Participation is voluntary. I understand that risks include falls, collisions, awkward landings, and movement or deflation of the inflatable. Injuries can include cuts, bruises, sprains, broken bones, and head or neck injuries. I have considered these risks before giving permission.
Limited release of the signing adult's own claims
In exchange for allowing the listed children to participate, and only to the extent Wyoming law permits, I release Heart Hopper Giants LLC from claims that I personally hold for injury, loss, or expense arising from their participation, including claims based on Heart Hopper Giants LLC's ordinary negligence. This release does not waive any child's legal rights. It does not release gross negligence, reckless, willful, or intentional misconduct, or any claim that the law does not allow to be waived.
