Fees Summary
Registration Fee: $90 + GST (one-time, non-refundable) — due at the time of registration to confirm your child's spot.
Monthly Tuition: $200 + GST — billed monthly from September to June (10 payments total) via automatic online payment.

Payment Method
Registration Fee is paid securely online immediately after completing this form.
Monthly Tuition is collected through an automatic online subscription. In September, before the program begins, you'll receive a secure payment link by email to set up your monthly billing for the school year (10 monthly payments, September–June).
By completing registration, you authorize Words in Motion to process your Registration Fee now, and to send your Monthly Tuition subscription link in September.
Withdrawal Policy
If a family needs to withdraw a child during the school year, one month's written notice must be provided by the 1st of the preceding month (e.g., notice given November 1st for a December 1st withdrawal), addressed to the Director.
If a child is withdrawn after January 31st, remaining fees for the year may be forfeited at the discretion of the Director.
Participation Release Waiver
Health Acknowledgment
As the Authorized Signatory of the listed participating student, I attest that I, and the Student, will:
- Not attend if experiencing symptoms of illness such as cough, shortness of breath, fever, chills, muscle pain, headache, sore throat, or new loss of taste or smell
- Not attend if we believe we have been exposed to someone with a suspected or confirmed case of COVID
- Not attend if diagnosed with COVID and not yet cleared as non-contagious by local doctors and public health authorities
Release of Liability
I hereby release and agree not to hold Life in Motion Education Inc., its directors, owners, employees, volunteers, host facilities, or others acting on their behalf, liable for any physical or mental injury, illness, death, or loss of personal property that may occur during or in connection with the Services. I voluntarily release and forever discharge Life in Motion Education Inc. from any and all causes of action connected with the Services, including with respect to COVID-19 and any bodily injury, illness, medical treatment, or property damage arising from participation. This release extends to all directors, partners, employees, locations, and volunteers.
Emergency Care Authorization
In the event of an accident or illness while in the care of Words in Motion staff, facilitators will make their best effort to contact the parent or guardian immediately. If unavailable, facilitators are authorized to secure appropriate care and will attempt to contact the emergency contact listed in this form. The parent agrees that where staff have acted in good faith to follow proper procedures, the organization shall not be held liable, and any such liability is expressly waived by the parent.

By signing below, I confirm that I have read and agree to the Program Guidelines, Fees, Payment Method, and Withdrawal Policy described above and on the program page. I authorize my child to participate in the Words in Motion program.
