Getting Ready for Camp
Come dressed for adventure! We recommend closed-toe athletic shoes (no flip-flops), comfy clothes that are okay to get a little messy, and a hat or cap. Please send a labeled, healthy snack and lunch (nut-free, no candy), plus a spill-proof water bottle — and don't forget sunscreen, though we'll have spray sunscreen on hand just in case. Toys from home stay at home — we've got the fun covered!
Health Acknowledgment
As the Authorized Signatory of the listed participating student, I attest that I, and the Student, will:
- Not attend if experiencing symptoms of illness (fever, persistent cough, sore throat, or other signs of contagious illness)
- Not attend if we believe we have been exposed to a suspected or confirmed contagious illness
- Follow all health and safety guidelines communicated by Words in Motion staff
Release of Liability
I hereby release and agree not to hold Life in Motion Education Inc., its directors, owners, employees, volunteers, host facilities, or others acting on their behalf, liable for any physical or mental injury, illness, death, or loss of personal property that may occur during or in connection with the Services. I voluntarily release and forever discharge Life in Motion Education Inc. from any and all causes of action connected with the Services. This release extends to all directors, partners, employees, locations, and volunteers. In the event of an accident or illness, facilitators will make their best effort to contact the parent immediately, and are authorized to secure appropriate care if the parent cannot be reached.
Photo and Video Consent
I consent to Words in Motion taking and using photographs/videos of my child for internal, educational, and promotional purposes, including classroom materials, marketing, the Words in Motion website and social media, and community outreach.
Outings Consent
I give permission for my child to participate in supervised outings to local parks and beaches within walking distance of the program site, accompanied and supervised by staff at all times.
Agreement
By signing below, I confirm that I have read and agree to the above, and that the information provided in this form is accurate.
