GT BJJ Club Adult Participant Waiver

Program and Eligibility I certify that I am at least 18 years old and an approved member of the Georgia Tech Brazilian Jiu-Jitsu Club. I voluntarily choose to participate in the GT BJJ Club program conducted by Odyssey Jiu-Jitsu, including sessions at Odyssey Jiu-Jitsu and Odyssey-led sessions at the Georgia Tech Campus Recreation Center or another approved program location.
Covered Activities Covered activities include Jiu Jitsu, NoGi grappling, wrestling, takedowns, throws, joint locks, chokes, sparring, drilling, conditioning, warmups, instruction, demonstrations, open training, use of mats or equipment, and entering or remaining in training and facility areas before, during, or after a program session.
Acknowledgment of Risks I understand that these activities are physically demanding and involve close bodily contact. Risks include falls, impacts, collisions, overexertion, cuts, bruises, sprains, strains, dislocations, fractures, concussion, choking injuries, illness or infection, property damage, permanent disability, paralysis, and death. These risks may result from my conduct, another participant, coaching or instruction, rescue or medical response, equipment, mat or facility conditions, or the acts or omissions of others, including ordinary negligence. Other risks may exist that are unknown or not reasonably foreseeable.
Voluntary Assumption of Risk Knowing the nature of these activities and their risks, I knowingly and voluntarily accept and assume all inherent and other risks of participation, whether known or unknown, to the fullest extent permitted by Georgia law. I am responsible for deciding whether to begin or continue participating and will stop and notify a coach if I believe a condition or activity is unsafe.
Health, Hygiene, and Safety Representation I represent that I am physically and medically able to participate or have obtained appropriate medical clearance. I will disclose relevant limitations to the instructor. I will not participate while ill, contagious, impaired, or affected by an injury or condition that makes participation unsafe. I will not train with a suspected contagious skin condition, including ringworm, staph, or impetigo, until appropriately treated and cleared. I agree to follow coach instructions, hygiene requirements, safety restrictions, and facility rules.
Release and Covenant Not to Sue To the fullest extent permitted by Georgia law, I release, waive, discharge, and covenant not to sue Kumite Martial Arts LLC d/b/a Odyssey Jiu-Jitsu, its owners, officers, employees, coaches, contractors, volunteers, agents, successors, and assigns (collectively, the “Released Parties”) for claims, demands, losses, damages, or causes of action arising from or relating to my participation in the program, including claims based in whole or in part on the ordinary negligence of a Released Party. This release does not apply to conduct that cannot lawfully be released.
Responsibility for My Conduct I am responsible for injuries, losses, or property damage caused by my intentional, reckless, unlawful, or materially rule-violating conduct. To the fullest extent permitted by law, I agree to indemnify and hold the Released Parties harmless from third-party claims arising directly from such conduct by me.
Emergency Medical Assistance If I am unable to provide consent during an emergency, I authorize the provision of reasonable first aid and emergency medical assistance, including contacting emergency services. I understand that Odyssey does not guarantee medical personnel or equipment will be available and that I am responsible for medical, ambulance, transportation, and related expenses incurred on my behalf.
Personal Property I am responsible for my personal property. The Released Parties are not responsible for loss, theft, or damage except to the extent responsibility cannot lawfully be waived.
Rules, Removal, and Reporting I agree to follow the GT BJJ Club Student Protocols and Rules, all coach instructions, and applicable Odyssey and Georgia Tech facility requirements. I will immediately report injuries, unsafe conduct, damaged equipment, and hazardous conditions. I understand that Odyssey may suspend or end my participation for safety, conduct, eligibility, or policy violations.
Duration and Locations This agreement applies to all GT BJJ Club program visits, classes, training sessions, and related activities conducted by or with Odyssey during the Fall 2026 program term, whether held at Odyssey, the Georgia Tech Campus Recreation Center, or another location approved for the program.
Severability and Georgia Law This agreement is governed by Georgia law. If any provision is held invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law.
Electronic Signature and Acknowledgment I have read this agreement completely, understand its terms, understand that I am giving up substantial legal rights, have had the opportunity to ask questions, and sign freely and voluntarily without inducement. By selecting “I Agree” and submitting this form through WaiverFile, I intend my electronic signature to have the same effect as a handwritten signature.
