I hereby give the Solid Waste Authority of Palm Beach County, its licensees, and legal representatives my express consent to use, publish, print, display or otherwise unconditionally use, at no cost, my child/ward’s name (or any fictional name), picture, portrait, photograph, visual, and/or voice recording and likeness, in all forms and media and in all manners, including photo, film, audio, video, and all social media representations, for nonprofit, public or educational purposes, and I waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith, and also expressly waive any claim or right to compensation for such use.
The undersigned hereby expressly represents that he/she is the parent or legal guardian of the minor child named herein and has the authority to execute this release on this child’s behalf and that this representation is specifically made and intended for the SWA to rely on this representation in all respects and for all purposes including, but not limited to, the purposes set forth herein.
