INFORMED CONSENT/PERMISSION FORM FOR EDUCATION TRIPS

(Students Under 18 Years)  

 

The school is arranging a visit to Blue Mountain Resort

THIS FORM MUST BE READ AND SIGNED BY EVERY STUDENT WHO WISHES TO PARTICIPATE AND BY A PARENT OR GUARDIAN OF A PARTICIPATING STUDENT.

ELEMENTS OF RISK:  

 Educational activity programs, such as Blue Mountain attractions    involve certain elements of risk.  Injuries may occur while participating in these activities.  The following list includes, but is not limited to, examples of the types of injury which may result from participating in one or more of: gondola, hiking, climbing wall, ropes courses or zip lines, wood ball track, net adventure, putting course, golf, alpine coaster, cycling, aquatic activities, paddle boats, water trikes, instructional type activities. (describe activity)

The risk of sustaining these types of injuries result from the nature of the activity and can occur without any fault of either the student, or the school board, its’ employees/agents or the facility where the activity is taking place.  By choosing to take part in this activity, you are accepting the risk that you/your child may be injured.

The chance of an injury occurring can be reduced by carefully following instructions at all times while engaged in the activity.

If you choose to participate in Blue Mountain attractions you must understand that you bear the responsibility for any injury that might occur.

The school board does not provide accidental death, disability, dismemberment or medical expense insurance on behalf of the students participating in this activity.

WE HAVE READ THE ABOVE.  WE UNDERSTAND THAT IN PARTICIPATING IN THE ACTIVITY DESCRIBED ABOVE, WE ARE ASSUMING THE RISKS ASSOCIATED WITH DOING SO.

High Ropes Courses (Grades 8-12)

Features various courses and more than 75 aerial elements including suspension bridges, ladders, cargo nets, zip lines and more. There are multiple levels of progression. A harness and helmet are provided. Closed toe and closed-back shoes are required. Running shoes are recommended. A weight restriction of 250 pounds applies. Must be 10 years of age or older and be able to reach 5 ft. for the Green courses and 6 ft. for the Blue courses with feet flat on the ground. 10 to 12 year old’s can access the Green and Blue Courses only. 10 to 12 year old’s must be accompanied by an adult. For Black Courses you must be 13 years of age or older and able to reach 6 ft. while keeping feet flat on the ground. Timber challenge high ropes does include zip line features.

 

The Zip lines (Grades 8-12)

Are three parallel lines that riders descend down the mountain on two sections each 400 ft. long from Timber Challenge to Basecamp, reaching heights of 50 ft. at speeds of up to 50 km/h.  Closed toe and closed-back shoes are required. Running shoes are recommended.  Must be 13 years of age or older. A weight restriction of 250 pounds applies. Individuals who are pregnant or have had kidney transplants are prohibited from participating on any ropes course or zip line.

 

Canopy Climb (Grades 1-12)

Has suspended rope nets, slides, tree houses and watchtowers. Closed-toe and closed-back shoes are required. Running shoes are recommended.  Guests aged 6-12 may access the attraction alone but must be supervised by an adult at ground level (Max 5 kids to 1 adult).

 

Cocoon Crawl Playscape (Grades 1-12)

A suspended net attraction with various levels through tunnels and loops. Climb 3 levels to the top of the crochet net attraction with bounce and fun! Ground play areas include a swing, unharnessed balance beam rope course and a spiders web. Guests aged 6-12 may access the attraction alone but must be supervised by an adult at ground level (Max 5 kids to 1 adult). Height and weight restrictions may apply. Shoes are not permitted on attraction. 

 

Lumber Lanes (Grades 1-12)

Climbers of all ages and abilities will find their route at this incredible outdoor climbing centre. 8 lanes with 8 unique climbs ranging from traditional climbing grips to wood and rope elements. Ages 3-12 must be accompanied by an adult (18+). Minimum weight is 40 lbs. (18 kg) Maximum weight is 250 lbs. (113 kg) Closed-toe and closed-heeled shoes are required for all participants. Harnesses provided; helmets available upon request. Participants must have sufficient physical dexterity and the ability to use the equipment properly. Participants who have undergone a kidney transplant or who are pregnant are prohibited from participating on any ropes course.

 

Chutes and Loops (Grades 1-12)

Roll a wooden ball through an interactive wooden fort with elements like cog and wheels, bells, tunnels, funnels, twists, and conveyer belts. Each run offers a unique adventure, providing endless family fun for little and big kids alike. Suitable for any age, designed for younger participants.

 

Open-Air Gondola (Grades 1-12)

360-degree open-air gondola passenger ropeway. Suitable for all ages. Must be 122cm (4ft) tall to ride alone. Maximum 8 persons per gondola. Pets are not permitted: Service animals are permitted, with proper documentation, please speak to the operator. Strollers, wagons, and wheelchairs are permitted with a size restriction of 81 cm x 112 cm (32" x 44"). Scooters, tricycles, skateboards, bicycles, etc. are not permitted. It is recommended that pregnant guests do not ride the gondola.

 

Cascade Mini Golf (Grades 1-12)

Designated for both kids and adults, this multi-level 18-hole, putting course offers a scaled- down version of bunkers and water hazards found on full-size courses.

 

Millpond (Grades 8-12)

An aquatic adventure with aqua trikes and water tricycles with big wheel paddles. Participants must be ages 5+ and weigh at least 30 lbs. (13.6 kg), Participants ages 5-12 must ride with an adult (18+) Participants ages 13+ can ride solo or together Max weight per trike is 400 lbs. (181.4 kg) Personal floatation devices are required and are provided.

 

Ridge Runner Mountain Coaster (Grades 8-12)

Must be 13 years of age or older to ride alone. Minimum height to ride alone is 137 cm (54in) tall. Maximum weight capacity is 150 kg (330 lbs). Suitable clothing and footwear must be worn.

RESPONSIBILITY CODE

There are elements of risk that common sense and personal awareness can help reduce. Please

adhere to the code listed below and share with others the responsibility for a safe experience.

 

1.   Observe and obey all posted signs and instructions from staff.

2.   Do not stop on track until finish area - except in case of emergency.

3.   Seatbelt must be worn at all times. Removal of seatbelt may result in serious injury or death.

4.   You must not use coaster if your ability is impaired through use of alcohol or drugs.

5.   You must control your speed keeping a safe distance behind the cart ahead. Tailgating is

      prohibited. If track is wet or icy you must increase the distance between carts due to longer

      braking distance.

6.   You must not collide with the cart ahead of you.

7.   Keep hands on both brake handles and arms and legs inside cart at all times.

8.   Face direction of travel at all times. Never turn around, lie down, kneel or stand.

9.   If ride stops you must remain in cart with your seatbelt fastened until ride starts again or you

      are instructed otherwise by staff.

10. If you are involved in or witness a collision or accident, you must remain at the unload area

      and identify yourself to attendant.

DANGERS AND RISKS INHERENT TO THE DIFFERENT RESORT ACTIVITIES:

 

The resort activities listed above and all other resort activities all involve certain elements of dangers and risks.  Injuries may occur while participating in these activities. Such dangers and risks include, but are not limited to:

 

Reduced oxygen in the air; rapidly changing weather conditions; exposure to the sun, rain, snow, hail and lightning; floods; landslides; open water hazards; lack of shelter; encounters with wildlife, insects or poisonous plants; natural or humanmade features or structures; steep inclines and declines; uneven and varying slopes and terrain; rocks of various sizes; trees, stumps, branches, and limbs; exposed roots; downed timber; cliffs; holes and potholes; surface depressions; wet and/or slippery surfaces and features; dirt features and other constructed features including bridges, ramps, ladders, bumps, berms, jumps, gaps, drops, wall rides and elevated stunts; negligent design and/or construction of features and trails; debris on tracks or trails; failure or malfunction of equipment including brake devices; trips, slips and falls; collisions with vehicles, structures, features, or other participants; flying disks, golf balls, or other objects; being hit by a drone; being hit by a gondola cabin or chair; loss of balance or control; falling objects; falling from platforms, cables, gondolas, chairlifts and any other conveyance or structure; climbing ladders or other vertical structures; falling off the wall or ropes; loose or damaged hand or toe holds; failure or malfunction of ropes, cables, or other gear or devices including broken ropes or cables, tethers, pulleys, and harnesses; becoming lost or separated from Resort staff or other participants; vertigo; water immersion, drowning, hypothermia; overexertion; dehydration; sun burns, sun stroke, skin burns and abrasions; cuts, infection, fatigue, strains, fractures, activation or aggravation of pre-existing physical injuries, conditions or congenital anomalies; and risks involved with rescue operations and/or administration of care, medical or otherwise, conducted by Resort staff.
